What is Structured Family Caregiving (SFC)?

The Structured Family Caregiving program (SFC) offers financial and support services to Georgia families that deliver care at home for a senior or disabled person.

SFC allows family members to care for their own family members. The family caregiver must live with the member/client. No matter if you are the daughter, son, cousin, aunt, niece, nephew or related by marriage, you may be eligible to care for your own family member and receive a stipend for the care you provide to your loved one.

FULL-SERVICE PROVIDER

CCSP and SOURCE

The Community Care Services Program (CCSP) is a Medicaid home and community-based waiver services 
program. *STATE ASSISTANCE*

Service Options Using Resources in a Community Environment (SOURCE) *Federal Assistance* Monthly Income of $943 or less.

As an experienced case manager for CCSP and SOURCE  affiliated with 16 years of service, I will ensure the completion of all necessary documentation to secure approval for the program.

The GA Division of Family & Children Services (DFCS)

Our team will assist with all application admission for AGED, BLIND AND DISABLED MEDICAID (ABD) with DFCS. 

Application with DFCS can take up to 45 days for Medicaid approval.  Savannah SFC Services will work with agency to seek approval.

SOURCE does not need the DFCS approval to receive SFC assistance.

PAID CAREGIVER ASSISTANCE

Once Medicaid or SOURCE approval is granted, services can commence at home. 

  • The caregiver is now eligible to receive financial assistance in the home.
  • A tax-free financial stipend.
  • A professional health coach to help you understand your loved one's condition.
  • Ongoing support including useful care tips and local resources.
  • Access to a secure mobile app to connect you with your care coach.
  • ​Peace of Mind knowing you are providing the care your loved one needs in the comfort of their home.

How it Works –

As a patient you are likely eligible if you…

  • Have Medicaid  
  • Live in Georgia in one of these counties: Bryan, Bulloch, Burke, Camden, Chatham, Columbia,Effingham, Liberty, Richmond, and Screven.  
  • Must meet Nursing Home Level of Care (need help with day-to-day activities)  
  • Able to direct your own care/services (if not, have an individual representative to appoint)  
  • Over 21-64 years of age with a recognized disability.

How it Works – Caregiver

Who can be a caregiver?

  • Someone who is at least 18 years of age 
  • Someone who eligible for work in the United States  
  • Someone who is not excluded from the Medicaid program
  • The caregiver must be a family member who lives with the Participant in the private home of the Participant or Caregiver. The caregiver cannot be a spouse.

About us

Scarlett Brown Founder/Owner 

Total of 16+ years of experience in DCH waiver services as a case manager for Community Care Services Program (CCSP) at the Chatham County Health Department, and 4 years in the same role at St. Joseph's Candler Georgia Infirmary, Scarlett has also been affiliated with the SOURCE program, another DCH waiver initiative.

Before joining CCSP, Scarlett dedicated 6 years to the Department of Family and Children Services as a Medicaid ABD/SNAP case manager. She is committed to implementing learning, development, and engagement programs that address caregivers' needs and contribute positively to the workplace environment. Ensuring that caregivers feel a sense of belonging, purpose, and meaning in their careers is paramount to her. Scarlett finds joy in life's simple pleasures, like being with family and friends, and has a deep passion for helping others, spreading joy and peace wherever she goes.

Alicia Johnson

Coaching Coordinator Manager

Johnson obtained her Bachelor of Science degree in Community Health from Georgia Southern University. She began her career as a Mental Health Clinician in the field of Mental Health, Developmental Disabilities, and Addictive Diseases in 2004. Her extensive experience includes supporting and caring for her son, who has Developmental Disabilities, Autism, and is Aging. This has endowed Johnson with a deep compassion for all individuals. She is well-known for her passionate advocacy on behalf of individuals with disabilities, both through the Supported Employment Program and as a volunteer mentor for the Effingham County Navigator Team's Young Adult Program.
